Enter the survey and questionnaire section as an individual or an organization to be qualified for membership-level placement. The information provided will be used to locate and identify candidates for Professional Consultation and Information Services while at the same time providing general benefits associated with PodzLyfe Membership. At the end of the form submission, you will submit your purchase to be placed on the relative and appropriate level for membership.

There are 12 individual levels and 10 organizational levels. 

The survey consists of personal, professional, and psychological questions with user inputs and multiple-choice selections. The form and data are encrypted and stored on a HIPPA compliance server for data integrity. The survey information will be used for spiritual guidance on the levels as well. The data provided will determine how we use spirituality through the levels of the PodzLyfe membership. For those who are looking to balance their spirituality, please state your issues in the survey and you will be placed on the respective level for consultation services and membership benefits. 

As an individual, here are a few items that will expedite your time taking the survey: Your resume for dates and other relative information, relative personal accounting, and enough time to answer the questions honestly and to your best abilities. There are about 48 sections to work through.

As a leader of the organization, here are a few items that will expedite your time taking the survey: Access to your organizational email. Relative business accounting. Corporate structure, and leadership trends. There are about 34 sections to work through.

The PodzLyfe team will work diligently through the data submissions. We will create trendlines for the levels and create distribution curves for multiple-perspective analysis. One of our goals will be to connect the non-obvious dots and bring clarity to a murky situation. Or, enjoy good trendlines and find pleasure in the process. Working through the data will provide insight on how to benefit the PodzLyfe members with the expertise of the PodzLyfe team. We will use this insight to travel to meet specific communities of the PodzLyfe members and share in-person events.
